You see two TC copies with the number [2]. The steps to reproduce:
1. Two TC instances were started.
2. The second one was minimized to tray using AllToTray tool.
3. One more TC instance was started.
AllToTray makes the window invisible, so it seems that TC looks only for visible windows of itself when assigning a number.

I don't know AllToTray, but I can not confirm this bug with TaskSwitchXP's minimize to tray feature.

I have made the 2nd TC invisible by some other tool and cannot confirm this. Although the second TC is hidden, the next instance is properly numbered.AllToTray makes the window invisible, so it seems that TC looks only for visible windows of itself when assigning a number.

TC uses FindWindow - if nothing can be found, TC cannot know about the other instances...

Strange, I cannot reproduce this on Vista even with TC 6.55 (which is on a screenshot from start post) or older, tried to hide second TC instance window or to "minimize" it to tray with TrayIt! and AllToTray, but 3rd TC instance then started with [3] in title as expected. Maybe this bug/misfeature shows itself only on Windows XP or older?
