Well, you haven't answered my question about thumbnails mode yet. If you don't use thumbnails mode (and disabled icons like Ghisler suggested), there shouldn't be any shell extensions involved. The only thing I can think of is to look in Windows Event Viewer if there's an entry about an application crash (it is most likely a crash after all).
