If I understood you well, it seems to work on my PC see this screenshot

And also try to activate the following radio button:
Configuration->Options->Display->Show symbols left to the filename->All associated +EXE/LNK(slow, 32-Bit only)

I had same problem but with FastStone ImageViewer. Solved it simply by going to Control Panel -> Folder Options -> File Types, then I found ICO file extension and did Restore. After that everything is ok in Total Commander.

Another solution is to place the cursor on an icon file (.ico) and choose files - associate - edit type - change icon, and then put
"%1"
including the double quotes as the icon file name.
