How do you create transparent icons for TC?

How do you create icons (.ico files) for the TC button bar? I use Paint to edit the pixels as a .bmp file, MS Photo Editor to set the transparent colour, and Irfan View to convert to .ico but I can only seem to get tranparency to work half the time.
