Rephrasing my earlier post. I have noticed that the tooltip for file names that are too long to fit in the listing behaves in an odd fashion. It takes focus as if it were a small window, if I click on it intending to click on the file underneath.
When this happens a) the title bar of TC turns grey, despite that for all intents and purposes I am still interacting with the main window, and no new application was launched, b) double-click to open a file fails if the first click registers on the file and the second one on the tooltip, which has appeared in the meantime.
This is different from Explorer and most standard applications, where tooltips can be clicked "through" as if they weren't there.

I don't think that in Explorer you can click "through" a tooltip, it simply hides it when you move cursor out from file instead.
But TC doesn't hide tooltip in such case but updates it with details for another focused file so you don't need to wait until tooltip appears again.
As I see, TC moves tooltip with some delay so it becomes possible to click it, and it causes your problem. Unfortunately I doubt that there is a way to solve it for now w/o disabling tooltips.

I can conform the lost focus, but mo matter if using custom tooltips for certain file types, or using a truncated filename,
the focus is only lost for maybe a quarter second at most, and returns almost instantly.
Maybe this is system dependent, or some TC ini setting is interfering.

Yes, new tooltips after the first one appear without delay. That is normal and expected on any toolbar, taskbar and in TC. I see that Explorer actually makes me wait.
My little "problem" however occurs without moving the mouse to a new file. I try to click on the file already under cursor. Sometimes I have to try doubleclicking 2 or 3 times to open a file, because the tooltip catches one of my clicks. Also it then looks like some new program started and again closed.
The tooltip disappears immediately in Explorer if I click on it, and the file receives my click.
(Until now, I used the rename box F2/shift-F6 to read overly long file names, and didn't have the tooltip function activated.)

That's strange. As I see, tooltips are always shown outside mouse position (below and more to the right) so I simply can't click it w/o moving mouse to another file.My little "problem" however occurs without moving the mouse to a new file. I try to click on the file already under cursor.
I can only click a tooltip if I move my mouse, and I need to be very fast because tooltip changes its position when I do it.
The tooltip with file type, size, date modified and properties from word documents ("win32-style tips") shows to the right and below mouse position. However, the tooltip with only the file name ("if truncated") is positioned precisely on top of the file name, covering it completely and extending to the right and left over other columns as far as needed.
I suppose I could use the bigger popup instead, except most of the time I don't need it because I see size and date right in the panel.

Well, I'm always use Win32-style tooltips so haven't seen your problem (when Win32-style tooltips are enabled, they are always shown for truncated names), and I don't know why someone should use old-style tooltips. However I am still able to enter a folder using double-click on old-style tooltip (if I disable Win32-style tooltips).
