Small & Low Res Custom Thumbs

Hello,
I am new here.
Please advise if I am in the wrong spot or can give tips on finding a solution.
I am a graphics person & it is driving me bonkers that my .icos that I had spent a lot of time on, come out tiny & low res, compared to folder icons.
I have searched the web & either cannot figure out how to fix this, or cannot find a solution.
I cannot post an image, I guess

If there is a solution, please explain it as I am new.
I have read about irfanview, but not sure if that will lead anywhere, but I do have it.

I hope that I am answering you correctly.
The icons are exported at 256
In TC it is set at 128
When I changed this to 256, the border got bigger, but there is still padding around the icon/thumb. I would like to see the icon fill the entire square, like the folders do.

It is not limited to just my icons, it is all icons, sorry.
Like Word docs, flash, 7z, pdf, etc.
Image thumbs & folders are perfect.
I see that folder icons are small only when empty.
