Currently Total Commander can create/edit comments using Crtl+Z only if commented item is on filesystem that TC can write.
I suggest to use internal database similar to one TC uses for storing thumbnails to store comments for items on read-only filesystems.
To write a comment to r/o fs item that already has comment in descript.ion format, TC may display descript.ion part in Ctrl+Z dialog as not-editable text, with ability to add a new text from a new line and store that part in internal db.
If the file/folder with comment from internal db is copied from r/o fs to writable one, comment may be converted to descript.ion/files.bbs format.
To detect that comment belongs to certain file/folder on removeble media (where path could be the same but files are different) TC may use media (drive) serial number, label or both.

of any external file system to assign the correct internal stored data.
